If you are not a provider, but are seeking Housing Stabilization Services or a provider referral, please call County case worker or Care Coordinator for assistance.

Eligibility requirements

• Receive Medical Assistance

• Experiencing Housing Instability

  • Currently experiencing homelessness

  • At risk of becoming homeless

  • Transitioning from a licensed setting

  • Eligible for MA waiver services

• Needs assistance due to their mental health condition to:

  • Communicate

  • Manage challenging behaviors

  • Make decisions

Provider Referral Information

Referrals for Housing Stabilization Services can be made by a Targeted Case Manager, Waiver Case Manager or Housing Services Consultant.
